Assisting with Your Housing Transition
At 1st Choice Connections, we assist veterans, disabled and low-income individuals with finding affordable and suitable housing. Navigating the relocation process on your own can be daunting, that's why we work together to ensure that you receive all the services you are entitled to, helping to make your housing transition efficient and pleasant.
1st Choice Connections works directly with the Washington State Department of Social and Health Services (DSHS) to find and connect you with all relevant partners for your care. Services authorized in the plan of care may include, but are not limited to arrangements with:
Pharmacies
Primary care physicians and nurses
Financial institutions
Utility companies
Housing providers
Caregivers
Legal representatives
Local transportation providers

1st Choice Connections works directly with the Washington State Department of Social and Health Services (DSHS) to find and connect you with all relevant partners for your care. Services authorized in the plan of care may include, but are not limited to arrangements with:

At 1st Choice Connections, we conduct pre-transition consultations to discuss your unique needs and goals, allowing us to gain a better understanding of how we can provide you with the best possible care and support.
Housing and Transition
We provide assistance throughout the entire relocation process, working closely with local housing authorities and other agencies to ensure a smooth transition for our clients. Support is tailored to the individual's unique needs and may include services such as assessing eligibility for financial assistance, organizing the lease, setting up utilities, providing necessary furnishings, etc.
Transportation
We locate suitable transportation options and networks to help individuals connect with resources in their new community.
Medical Accessibility
We coordinate with local medical providers, such as caregivers, pharmacies, emergency providers and primary care physicians, to ensure that our clients' medical and accessibility needs are appropriately met in the new home.
Documentation
We also offer assistance with completing necessary paperwork and obtaining all required documentation for the relocation process, including the change of address.
