We assist eligible individuals who are working with a Department of Social and Health Services (DSHS) Case Manager and are seeking to transition into better housing for an improved quality of life. Our services are available to veterans, those with disabilities, low-income individuals, and the homeless.
How to Access Services
Eligible individuals will be accepted into the CCG-assisted transition program upon approval by a Washington State Department of Social and Health Services (DSHS) Case Manager. If you already have a DSHS Case Manager, contact them and ask to participate in the housing transition program with 1st Choice Connections!
THE FOLLOWING GUIDELINES ARE FOR YOUR REFERENCE:
To qualify for CCG services, individuals must meet the following criteria:
Be financially and functionally eligible for Medicaid long-term-care services, as assessed by a Washington State Department of Social and Health Services (DSHS) Case Manager*
Demonstrate a risk of housing instability or in need of housing for a lower level of care (e.g., moving from an assisted-living facility to an independent apartment)
To begin services, contact your DSHS Case Manager and ask about the opportunity to participate in the CCG-assisted housing relocation program with 1st Choice Connections.
If you do not know how to contact your Case Manager, call the Washington State DSHS Community Services Office (877) 501-2233
If you are not enrolled in Medicaid and do not have a Case Manager, begin an application with DSHS here
*Independent Payment Services
